How to Apply
Applications to the Family Medicine Residency Program at Baptist Memorial Hospital-DeSoto must be submitted through the Electronic Resident Application Service (ERAS) through the AAMC. Requests submitted outside of ERAS will not be accepted. Applications must include the below items including certified or notarized English translations of documents originally written in a language other than English.

Exam Scores
All applicants must complete their USMLE or COMLEX exams and report their Step 1 and Step 2 scores through ERAS to ensure that the program can download the results of these exams before the NRMP rank list deadline in mid-February. All applicable test scores must be submitted. If you receive an appointment, official copies will be obtained.
Curriculum vitae
The CV is an essential part of your application. Be sure to include your activities with professional societies, publications and any other relevant information about your education or experience.
Personal Statement
A personal statement with a summary of professional goals is required.
Medical School Diploma
A copy of your medical school diploma is required for all medical school graduates. If you receive an appointment, verification of completion will be obtained.
Dean’s letter
A thorough Dean’s Letter is required.
Medical School Transcripts
Current or final transcripts are required. These must indicate degree earned and date conferred, if applicable, from each medical school you attended. If you receive an appointment, official copies will be obtained.
Valid ECFMG Certificate
Certification is required if you graduated from a medical school outside the U.S. or Canada.
Three Original Letters of Recommendation
Submit 3 letters of recommendation from physicians or faculty members who are thoroughly familiar with your work. If you have had prior residency training, a letter from your program director is required. Each letter should include the writer’s opinion of your professional, academic and personal qualifications as well as an opinion of your potential success in the field to which you are applying.

Personal interviews are by invitation only and are based on the applicant’s academic performance, United States Medical Licensing Examination (USMLE) scores, * letters of recommendation and other accomplishments as detailed in the ERAS application.
All appointments are made through the National Resident Matching Program (NRMP). You must register through the NRMP website in addition to applying through ERAS.
